This work proposal includes only a visual inspection of the tree to determine the scope of the work and shall not be considered a tree risk evaluation. Risk evaluations involve more extensive inspection and are conducted as a separated work item at an additional charge.

The above prices, specifications and conditions are satisfactory and are hereby accepted. Jerry’s Tree Service is authorized to do the work as specified. Payment is due in full upon completion of all above described work. Unpaid balances will be assessed 1 / 2 % per month(18% APR) Finance Charge. Customer will be responsible for all collection expenses to include but not limited to court costs, attorney fees, collection fees, penalties and interest.
